The CCC Care and Support Team offers practical, compassionate assistance to church members and families experiencing illness, bereavement, crisis, isolation, or another significant need.

Working under pastoral leadership, the team listens, checks in, coordinates approved support, and helps connect people with pastoral care or appropriate community services. We serve with prayer, wisdom and respect for every person’s dignity and privacy.

How We Support Our Church Family

  • Receive care needs through approved church channels and confirm what support is requested.
  • Offer appropriate check-ins, visits, meals, transportation or other practical help when authorized.
  • Coordinate with pastoral leaders when spiritual care, crisis response or further follow-up is needed.
  • Maintain appropriate, limited records of assigned support and completed follow-up.
  • Keep a current list of approved church and community referral resources.

Our Ministry Rhythm

  • Weekly: Review assigned care needs and complete agreed check-ins.
  • Monthly: Meet for prayer, resource review and ministry training.
  • Urgent needs: Notify the designated pastoral leader and follow the approved response process.
